^Wisconsin 128 starts out toward the south from its northern terminus at Wisconsin 64, July 2025.
Wisconsin Highway 128 is a fairly short north-south highway in the "Indian Head" of western Wisconsin. The northern end of Highway 128 is at Wisconsin Highway 64 north of Glenwood City. From here, the route goes south into Glenwood City, where it meets the western end of Wisconsin Highway 170. Beyond this point, Wisconsin Highway 128 continues south on an alignment that is roughly due-south, crossing US Route 12 near Wilson. East of Spring Valley, Highway 128 crosses Wisconsin Highway 29. Finally, Wisconsin Highway 128 reaches its southern endpoint at Wisconsin Highway 72 in Elmwood.
