Ohio State Route 118 is a north-south route from Van Wert to Greenville in west-central Ohio. As US Route 127 also serves these two towns, Ohio Route 118 can be considered a low-key alternative to US Route 127. The central intersection to Van Wert is where Ohio Route 118 begins at US Route 127, and it heads southwesterly at first. Soon, in Rockford, it will turn southerly to parallel US Route 127. It will meet US Route 33 there as well. Farther into Mercer County, Ohio Route 118 serves more towns than US Route 127, only a few miles to the east. While US Route 127 serves Celina, Ohio Route 118 passes through Coldwater and St. Henry on top of Rockford. At the county line with Darke County, Ohio Route 118 meets Ohio Route 319, a tiny spur west to Burkettsville. In Darke County, the route serves New Weston, Rossburg and Ansonia before reaching its final destination of Greenville. The southern end of Ohio Route 118 occurs at the central traffic circle in downtown Greenville, just after Ohio Route 49 and Ohio Route 571 pick it up and take it there. That traffic circle also features appearances from Ohio Route 121 and Ohio Route 502.

The guide assembly shown explains that continuation along US Route 33 eastward requires a turn toward the south along Ohio Route 118.
