Ohio State Route 345 is a short route southwest of Zanesville, connecting New Lexington and East Fultonham. Most of Ohio Route 345 is in Perry County, but the northeastern end, at US Route 22, is in Muskingum County. From this northeastern end in East Fultonham, Ohio Route 345 goes south-southwesterly to New Lexington. It parallels Ohio Route 93 to the west. The southern end in New Lexington is at Ohio Route 13 and Ohio Route 37, which share a concurrency.
