You'll only find US Route 422 in two states, and the westerly of these two states is Ohio. When it's in Ohio, as opposed to Pennsylvania, US Route 422 is working its way toward or from the Public Square in downtown Cleveland, where it begins (or ends). (The Public Square is also traversed by US Route 6 and US Route 20, and it serves as a terminus for Ohio Route 3, Ohio Route 8, Ohio Route 14, US Route 42 and US Route 322.) From the Public Square, US Route 422 heads southeasterly on Ontario Street, then easterly on Woodland Avenue. It continues southeasterly on Kinsman Road, then easterly on Chagrin Boulevard. At Interstate 271, US Route 422 jumps southerly to take over the east-west freeway that comprises most of Interstate 480. US Route 422 uses that freeway east for a short distance, before that freeway ends. US Route 422 continues east to Parkman, then southeast to Youngstown, by way of Warren. The northern part of Youngstown's inner beltway carries US Route 422 after it trades places with Ohio Route 193. US Route 422 leaves Youngstown to the east, crossing into Pennsylvania near New Bedford, PA.

The trailblazer depicted reassures drivers that they turned toward the northwest along US Route 422 (unless that wasn't their intended plan of action!).
