Ohio State Route 646 runs east-west in Jefferson and Harrison Counties, in east-central Ohio. The western end is at US Route 250 alongside the waters of Tappan Lake. From this terminus, Ohio Route 646 runs northeast to Scio, where it meets Ohio Route 151. At Scio, Ohio Route 646 straightens out somewhat into an easterly direction. Later on, the route enters Jefferson County to pass to the south of Richmond, Ohio. Soon after that, Ohio Route 646 ends at Ohio Route 43 on the outskirts of Wintersville, just barely north of US Route 22. Many of the travelers heading east at Ohio Route 646's eastern end will join US Route 22 to continue east.

The guide assembly shown serves northbound travelers on Ohio Route 43 who just left the interchange with US Route 22. The signage shown sits on the northeast corner of the intersection between Ohio Routes 43 and 646, pointing out the location of the left turn onto Ohio Route 646.
